You have the choice of filling in the Standing Order Mandate with your bank details or setting up payment yourself (ticking the appropriate box on the form) using on-line banking if you would prefer (using the club’s bank details that the form shows), but please only do the latter once your application has been accepted.

Please also help the Derby RFC Membership Secretary (VPs) to know more about you (in case he does not know you) by filling in the box showing your connection with the club. Example entries might be – “M&J Parent”, “Sponsor”, “Supporter”, “attended as guest of Sponsor”, “former player” etc….
